Transition
Support
SAFEGUARD
Adolescent Services transition support plan consists of the following
components:
-
An assessment of the family and child needs. This will include
interviews with parents, a review of the records and phone interviews
with case manager at current residential facility. The outcome
of this process will be to clearly define target transition support
strategies.

The implementation of a transition support plan that identifies
specific support strategies.

Consultation with the family, the transition counselor and family
therapist to support implementation of the transition plan.
Duration
and Intensity of Program
SAFEGUARD
provides two base program options for families. The can also be
individualized to meet the families needs.

3 hours per day, 2 day per week supervision and implementation
of transition plan by the transition counselor. The case manager
will supervise the implementation process. The case manager will
provide in home consultation one time per week.

Following
each four-week interval the plan will be assessed and modified by
the transition team.

Adolescent Services subscribes to the TEAM approach to transition
support. The transition TEAM consists of the following components:
- The
Family
The transition support plan is an assessment that includes the
family in the decision making process. Several recommendations
are made to the family that make adjustments in their schedule,
interactions and household rule following procedures. The recommendations
are designed to provide the transitioner an environment for success.
It is recommended that the family be open to suggestions and consultation
in these areas.

Case Manager
SAFEGUARD
Adolescent Services case managers are masters degree level professionals
who have 5 or more years experience consulting with families and
implementing transition and behavior plans. The case manager presents
the transition plan to the family and transition counselors. The
case manager provides consultation for the family in environmental
changes and interaction strategies. The case manager provides
weekly in home and as needed phone consultation for the family
and transition counselor in implementation of problem solving,
conflict resolution and goal setting strategies.

Transition Counselor
SAFEGUARD
Adolescent Services transition counselors professionals who have
extensive experience implementing transition and behavior plans.
The transition counselor receives periodic crisis and behavioral
implementation training. Two transition counselors will be assigned
to each transition case. The family will have the opportunity
to meet the transition counselors prior to transition. The transition
counselor provides supervision of the transitioner during skills
training and recreational activities. In addition, provides transport
to and from support group meetings as well as crisis consultation
with the family. The transition counselor shapes, models and cues
appropriate problem solving, goal setting and conflict resolution
skills.
